Autumncare Health Services has a dedicated and compassionate hospice team who will aid the patient and their loved ones throughout the entire process. Most of the time, hospice care is provided at home or a care facility with family members acting as primary caregivers. The hospice care team visits regularly and help manage the care plan. This way, stress and apprehension are minimized and the quality of care is improved.
The team is composed of many members. Some may see the patient more than the other but that doesn’t make them less important than the other. The hospice team serves as a support system, with the primary physician serving as the head. The hospice care may consist of the following:
- Licensed Nurses and Nursing Assistants
- Personal Caregivers
- Social Workers
- Spiritual Coordinators
- Bereavement Specialists
Each member contributes in improving the disposition of the patient and helping him/her along with loved ones to achieve acceptance and peace.
